    Hi,

    I  have a new (very short) phantom limb pain survey on the following website as I am researching a book on PLP and sports. I have had quite a few Adaptive sports people complete the survey already but would like more. You don't have to do sports to take part, it is for anyone who is an amputee (whether they suffer pain or not). The results will be available to everyone including the medical establishment assuming I recieve enough responses for it to be a good cross section of amputees.

    Large Discounts on my products (book and CD's etc) will be available to those who leave me a contact email address once the book is published. The survey is completely anonomous. There are only a few mulitple choice questions so please pop along and leave your responses.
